Bring to Registration

§         Birth Certificate – a copy is required for all new players.

§         Insurance – carrier and policy numbers needed.

§         USA Hockey Registration Receipt – for ALL players and coaches

§         Coaches – USA Coaching Card, Driver’s License, Background Screening

Payment Policy

Payment in full or 50% of the registration fees at the time of registration and a check post dated for December 1, 2008 for the remaining balance.  All registration fees must be paid in full by December 31, 2008.  Returned checks will be assessed a fee of $25 plus all bank fees.  After December 31, 2008, players with outstanding balances are restricted from district play.

Late Registration

After September 11, 2008, a late fee of $50 will be assessed.

Concession Stand Hours

There is a concession stand commitment for parents of 7 hours per player. To opt out of concession hours, $90 per player must be paid at the time of registration.

 Referees

A Referee Clinic will be held at a date and time to be announced.  Referees must attend a clinic every year.  More information will be available at registration.

Coaches are Needed!

If interested, sign up at registration or contact the ACE Director, John Erickson at 482-5969, prior to registration.  More information on USA Hockey Coaching Clinic dates and locations will be available at registration.  Pre-registration is required for the clinics.  COACHES…Don’t forget your coaching cards!!!

Used Equipment Sale

The CCJHA Auxiliary will conduct a Used Equipment Sale at registration.  Those with equipment to sell can bring their priced items starting at 5:30PM.  The sale opens at 6:30PM.  NO EARLY SALES!  The CCJHA Auxiliary receives 15% of the selling price of all equipment sold.
